Community Service Verification

If you would like to request a letter of verification for community service hours completed with
Lifted with Love Inc., please read our requirements and fill out the form below!
Please allow up to 5-7 days for your verification letter to be completed and returned.  Thank you! 

Requirements -  
1. A minimum of 20 Hours served with Lifted with Love Inc. 
2. Community service hours must be within a 12 month time span of your verification request. 
3. Must provide valid proof of participation, donations do not count. (pictures, individual project letters, dates etc.) 
4. Contact information of individual, group or organization that letter will be sent to. 
*If being sent to an organization or school please add direct link to application, etc. 

All verification hours included in letter will be hours served with Lifted with Love Inc. only. No outside hours will be included. If you do not meet the minimum requirements for a verification letter your request will be denied. If any information is falsified, your request will be denied.

If any illegitimate letters are written on behalf of Lifted with Love Inc. legal actions will be taken. 

All official letters will be signed, stamped and dated on letterhead.
